Why Do My Hands Shake During Intermittent Fasting?

Shaking hands during intermittent fasting is usually caused by a drop in blood sugar, low electrolytes (especially sodium, potassium, and magnesium), or a surge of adrenaline as your body switches fuel sources. In most healthy people it is temporary and harmless, and it fades as you adapt to fasting over one to three weeks.

Why This Matters

Tremors are frightening. Your hands shake while you are holding a cup, your fingers feel unsteady, and your first thought is that something is seriously wrong. For most people fasting for 16 or 18 hours, nothing is wrong — but the shaking is a real signal that deserves attention rather than being ignored.

The reason it matters is that the same symptom can come from a harmless adaptation or from genuine hypoglycemia in someone taking diabetes medication. Knowing the difference tells you whether to keep going, adjust your approach, or stop and eat. It also matters because most people quit fasting after their first shaky morning — when a small change in salt or water intake would have solved it entirely.

What Actually Causes the Shaking

Falling blood glucose. During the first weeks of fasting, your body is still heavily dependent on glucose. As stored liver glycogen runs down — usually around hour 12 to 16 — blood sugar dips. The nervous system reacts by releasing adrenaline and noradrenaline to push glucose back up, and one of the most visible side effects of adrenaline is a fine tremor in the hands.

Electrolyte depletion. This is the most underestimated cause. When insulin falls during a fast, the kidneys excrete more sodium and water. Losing sodium pulls potassium and magnesium out of balance too. Low magnesium in particular causes muscle twitching, tremor, cramping, and eye-lid flickering. Many people who think they have a blood sugar problem actually have a salt problem.

Caffeine on an empty stomach. Black coffee does not break a fast, but coffee on an empty stomach hits far harder than coffee after food. Caffeine amplifies the adrenaline response already happening during a fast. Three cups of strong tea or coffee before noon is one of the most common reasons for fasting tremors in people who otherwise feel fine.

Dehydration. Fasting reduces total water intake because roughly 20 to 30 percent of daily fluid normally comes from food. Less blood volume means less stable circulation and a shakier feeling overall.

Poor sleep and stress. A body that is already running on high cortisol from a short night will produce tremors much earlier in a fast than a well-rested one.

When it is not normal. If you take insulin, glibenclamide, or other sulfonylurea medications, shaking during a fast can mean true hypoglycemia. The same applies if the tremor comes with cold sweat, confusion, blurred vision, a racing heart, or near-fainting. That combination means stop fasting, eat something, and speak to your doctor before fasting again.

Practical Tips

  • Add salt. Half a teaspoon of salt dissolved in water, or a cup of clear broth, during your fasting window. This is the single fastest fix and it does not break a fast.
  • Supplement magnesium. 200 to 400 mg of magnesium citrate or glycinate in the evening. Give it a week before judging.
  • Cut caffeine in half. Especially before noon. Never drink coffee as the first thing after waking during a fast.
  • Drink water steadily, not all at once. Two to three liters spread through the day, with salt, works better than plain water alone.
  • Shorten your fasting window temporarily. Drop from 18 hours to 14 or 16 for two weeks. Adaptation is a process, not a switch.
  • Eat enough in your eating window. Chronic under-eating is a hidden cause of tremors. If you are eating 900 calories a day in one meal, the problem is not the fast — it is the deficit.
  • Break your fast if symptoms escalate. A tremor with confusion or cold sweat is not something to push through.

Frequently Asked Questions

How long do fasting tremors last?

For most people the shaking appears in the first one to three weeks of consistent fasting and then disappears as the body becomes more efficient at burning fat and producing ketones. If tremors are still happening after a month, the cause is more likely electrolytes, caffeine, or under-eating rather than adaptation.

Does salt water stop shaking hands during a fast?

Often, yes — and quickly. Sodium loss is one of the main drivers of fasting tremors, and a glass of water with a pinch of quality salt frequently settles the shaking within 15 to 30 minutes. If salt makes no difference at all, blood sugar or caffeine is the more likely cause.

Is shaking during fasting dangerous?

In a healthy person without diabetes, occasional mild tremor is uncomfortable but not dangerous. It becomes a genuine concern when it appears alongside confusion, cold sweat, blurred vision, or fainting, or in anyone using blood-sugar-lowering medication. In those cases, break the fast immediately and consult a doctor.

Can drinking coffee make hand tremors worse while fasting?

Yes, significantly. Caffeine stimulates adrenaline release, and during a fast your adrenaline is already elevated. Coffee on an empty stomach can turn a mild, unnoticeable adaptation into visible shaking. Reducing to one cup, and drinking it later in the fasting window, usually resolves it.
